GStreamer Google Summer of Code: Chapter Support

Implement chapter support in more GStreamer plugins.

There is some preliminary code in MP4 supporting chapters, and the Matroska plugin handles the Table of Contents. This is a good starting point and a good reference for the rest of the plugins.

A user-facing working sample solution will be implemented inside the Totem player.
